Why do I feel hypothyroid when my labs are "normal"?

Most conventional screening starts and stops with TSH — a pituitary hormone that signals the thyroid. TSH is a useful number, but it's an indirect one. It tells you what your brain is asking the thyroid to do, not how much usable hormone actually reaches your cells. Several things can leave you symptomatic with a "normal" TSH: your body may not be converting storage hormone (T4) into the active form (T3) efficiently, an autoimmune process may be simmering, or your personal "normal" may sit at an unhelpful spot within a wide population range.

What does a complete thyroid panel actually include?

Testing TSH alone is like judging a car by the dashboard light. A fuller evaluation typically looks at:

  • TSH — the pituitary's signal to the thyroid.
  • Free T4 — the main storage form of thyroid hormone.
  • Free T3 — the active hormone your cells actually use.
  • Reverse T3 — a "brake" that can rise with stress and illness.
  • TPO & thyroglobulin (Tg) antibodies — markers of autoimmune thyroid activity.

Could this be Hashimoto's?

Hashimoto's thyroiditis is an autoimmune condition in which the immune system gradually attacks the thyroid — and it's the most common cause of an underactive thyroid in the United States. What surprises many people is the timeline: antibodies can be elevated, and symptoms present, years before TSH drifts out of range. The National Institute of Diabetes and Digestive and Kidney Diseases notes that thyroid antibodies are present in most people with Hashimoto's — yet antibodies aren't part of a standard screen unless someone thinks to order them.

"Normal range" vs. "optimal": what's the difference?

Lab reference ranges are statistical — they describe where most people in a testing population fall, not where you feel and function best. Two results can both be flagged "normal" while one person feels fine and the other feels awful. A functional evaluation reads your numbers in the context of your symptoms, your history, and how the markers relate to one another, rather than treating a single value as a pass/fail gate.
